AllCare Health Alliance is an organization of Cooper University Health Care primary care physicians who provide enhanced, high-quality, and coordinated health care to patients with Fee-for-Service Medicare (Original Medicare) in South Jersey. Our physicians and care teams, working together with Medicare, share information about you and your specific needs to keep you healthy and treat you effectively when you are ill.

As an AllCare Health Alliance Fee-for-Service Medicare patient, you will receive more coordinated care from a team of doctors, nurses, and other professionals who develop individualized treatment plans for you to achieve and maintain wellness. You are at the center of the AllCare Health Alliance team. This personalized, more coordinated approach to care allows you and your family to have an active voice in your health care decision making.
We are committed to providing you with an excellent health care experience by helping guide you through all the care you need. We help you with referrals to specialists, scheduling follow-up visits, and any questions you have on your path to better health.

AllCare Health Alliance LLC is an Accountable Care Organization based in Camden, New Jersey, and operates under an agreement with the Centers for Medicare and Medicaid Services (CMS) in the Medicare Shared Savings Program (MSSP). We are dedicated to providing high-quality, cost effective health care to Medicare patients across southern New Jersey.
